CCS Eligibility
  1. Dane County Residence
  2. Eligible for Medical Assistance
  3. Have a mental health  or substance use diagnosis
  4. Have a functional limitation in or more major life activities caused by mental health or substance use issues as measured by the state screen.
  5. Need for psychosocial rehabilitation services
  6. Must have current prescription.​
  7. Dane County Department of  human Services
